ABSTRACT

ABSTRACT Objective: Recently, there have been reports of children with severe inflammatory syndrome and multiorgan dysfunction associated with elevated inflammatory markers. These cases are reported as presenting the Multisystem Inflammatory Syndrome in Children (MIS-C) associated with COVID-19. In this study, we describe with parental permission a case of MIS-C in an infant with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Case description: A seven-month-old infant, with SARS-CoV-2 infection and a history of extreme preterm birth and very low weight at birth, with an initial course of mild respiratory symptoms and abrupt progression to vasoplegic shock, myocarditis and hyperinflammation syndrome, shown by high levels of troponin I, ferritin, CRP, D-dimer and hypoalbuminemia. Despite the intensive care provided, the child developed multiple organ dysfunction and died. Comments: Patients with a history of extreme prematurity may present with MIS-C in the presence of COVID-19 and are a group of special concern.


RESUMO Objetivo: Recentemente, foram descritos relatos de crianças com exame positivo para o coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2) associado à disfunção de múltiplos órgãos, secundária à hiperinflamação, denominada de síndrome inflamatória multissistêmica pediátrica (do inglês multisystem inflammatory syndrome in children - MIS-C). O objetivo deste relato é descrever um caso de MIS-C em lactente com infecção por SARS-CoV-2 e com evolução fatal abrupta, a despeito do suporte de terapia intensiva pediátrica. Descrição do caso: Lactente de sete meses, com infecção por SARS-CoV-2 e antecedentes de prematuridade extrema, com quadro inicial de síndrome gripal e progressão abrupta para choque vasoplégico, miocardite e síndrome de hiperinflamação, evidenciados por níveis elevados de troponina I, ferritina, proteína C reativa (PCR), dímero D e hipoalbuminemia. Não obstante o suporte de terapia intensiva instituído, a criança evoluiu com disfunção de múltiplos órgãos e morte. Comentários: Pacientes com antecedentes de prematuridade extrema podem apresentar MIS-C na vigência de doença do coronavírus 19 (COVID-19) e constituir um grupo de preocupação especial.

ABSTRACT

RESUMO Apresentamos o caso de uma paciente de 71 anos de idade admitida à unidade de terapia intensiva clínica em condição de falência de múltiplos órgãos. Após o quarto dia, com aplicação de todas as modalidades de suporte à vida (estímulo com vasopressores, ventilação mecânica, terapia com diálise contínua, terapia com antibióticos de amplo espectro e outras medidas de suporte), dados inespecíficos fornecidos por acompanhante revelaram que a paciente vinha apresentando dificuldade persistente para deglutir líquidos e alimentos sólidos por alguns dias antes da admissão ao hospital. Após a realização de procedimentos adicionais de diagnóstico radiológico e endoscópico, detectou-se a presença de um corpo estranho no esôfago: um fragmento de fio de aço com cerca de 6 cm de comprimento e dobrado ao meio, que havia penetrado a parede do esôfago e se projetava ao nível da sétima vértebra cervical. Conseguimos remover o corpo estranho por via endoscópica sem maiores complicações e, a seguir, estabilizar nossa paciente, com utilização de medidas terapêuticas adicionais, conforme necessário.


ABSTRACT We present the case of a 71-year-old patient who was admitted to the medical intensive care unit in a state of multiple organ dysfunction. After the fourth day of applying all needed life-saving measures (vasopressor stimulation, mechanical ventilation, continuous dialysis treatment, broad spectrum antibiotic therapy, and other supportive measures), nonspecific heteroanamnestic data revealed that the patient had been having a persistent difficulty in swallowing liquids and food for a few days prior to hospital admission. After performing additional radiological and endoscopic diagnostic procedures, a foreign body was detected; a steel wire that had a length of approximately 6cm and was bent in a half had penetrated the esophagus and was projected into the seventh neckline. We managed to evacuate the foreign body endoscopically without further complications, and we stabilized our patient using additional therapeutic measures as needed.

ABSTRACT

The aim of this study was to discuss the safety and efficacy of regional citrate anticoagulation (RCA) on continuous blood purification (CBP) during the treatment of multiple organ dysfunction syndrome (MODS). Thirty-five patients with MODS were divided into two groups: the local citrate anticoagulation (RCA) group, and the heparin-free blood purification (hfBP) group. The MODS severity was assessed according to Marshall's MODS score criteria. Blood coagulation indicators, blood pressure, filter lifespan, filter replacement frequency, anticoagulation indicators, and main metabolic and electrolyte indicators were analyzed and compared between RCA and hfBP groups. RCA resulted in lower blood pressure than hfBP. The filter efficacy in RCA treatment was longer than in the hfBP group. The blood clearance of creatine, blood urea nitrogen and uric acid was better in the RCA group. RCA also led to higher pH than hfBP. Neither treatment resulted in severe bleeding events. In addition, MODS score was positively correlated with prothrombin time and activated partial thromboplastin time but negatively correlated with platelet concentration. RCA is a safer and more effective method in CBP treatment; however, it could also lead to low blood pressure and blood alkalosis.

ABSTRACT

ABSTRACT Objective: To summarize the effect of mild hypothermia on function of the organs in patients with multiple organ dysfunction syndrome after cardiopulmonary bypass surgery. Methods: The patients were randomly divided into two groups, northermia group (n=71) and hypothermia group (n=89). We immediately began cooling the hypothermia group when test results showed multiple organ dysfunction syndrome, meanwhile all patients of two groups were drawn blood to test blood gas, liver and kidney function, blood coagulation function, and evaluated the cardiac function using echocardiography from 12 to 36 hours. We compared the difference of intra-aortic balloon pump, extracorporeal membrane oxygenation rate and mortality within one month after intensive care unit admission. Results: Among the 160 patients, 36 died, 10 (11.24%) patients were from the hypothermia group and 26 (36.6%) from the northermia group (P <0.05). In northermia group, 45 (63.38%) patients used intra-aortic balloon pump and 4 (5.63%), extracorporeal membrane oxygenation; in hypothermia group, 35 (39.32%) patients used intra-aortic balloon pump and 2 (2.25%), extracorporeal membrane oxygenation( P <0.05). The patients' heart rate decreased significantly in the hypothermia group. The heart rate of hypothermia group is significantly slower than the northermia group at the 36th hour (P <0.05). But the mean arterial pressure of hypothermia group is significantly higher than the northermia group at the 36th hour (P <0.05). In hypothermia group, PO2, SvO2 and lactate were improved significantly compared to pre-cooling (P <0.05), and they were significantly better than the northermia group at the 36th hour (P <0.05%). Prothrombin time and activated partial thromboplastin time have no significantly difference between the two groups (P >0.05). But the platelet count has significantly difference between the two groups at the 36th hour (P <0.05). The aspartate transaminase, alanine transaminase and creatinine were improved significantly in the hypothermia group, and they were significantly better than the northermia group (P <0.05). Conclusion: Mild hypothermia is feasible and safe for patients with multiple organ dysfunction syndrome after cardiopulmonary bypass surgery.

A pesar de los avances en el desarrollo de las terapias de reemplazo renal, la mortalidad de la falla renal aguda permanece elevada, especialmente, cuando se manifiesta simultáneamente con fallas orgánicas distantes, como es en el caso del síndrome de distrés respiratorio agudo. Se revisa la relación bidireccional deletérea entre pulmón y riñón, en el escenario de disfunción orgánica, la cual presenta aspectos clínicos relevantes de conocer. Se discuten los efectos renales del síndrome de distrés respiratorio agudo y del uso de la ventilación mecánica a presión positiva, siendo el daño inducido por este (ventilator induced lung injury) uno de los modelos utilizado frecuentemente para el estudio de la interacción pulmón-riñón. Se enfatiza el rol de la falla renal inducida por la ventilación mecánica (ventilator-induced kidney injury) en la patogenia de la falla renal aguda. Asimismo se analizan las repercusiones pulmonares de la falla renal aguda, reconociéndose que esta condición patológica induce un incremento en la permeabilidad vascular pulmonar, inflamación y alteración de los canales de sodio y agua del epitelio alveolar, entre otros efectos. Este modelo conceptual puede ser la base para el desarrollo de nuevas estrategias terapéuticas a utilizar en el paciente con síndrome de disfunción orgánica múltiple.


Despite advances in the development of renal replacement therapy, mortality of acute renal failure remains high, especially when occurring simultaneously with distant organic failure as it is in the case of the acute respiratory distress syndrome. In this update, birideccional deleterious relationship between lung and kidney on the setting of organ dysfunction is reviewed, which presents important clinical aspects of knowing. Specifically, the renal effects of acute respiratory distress syndrome and the use of positive-pressure mechanical ventilation are discussed, being ventilator induced lung injury one of the most common models for studying the lung-kidney crosstalk. The role of renal failure induced by mechanical ventilation (ventilator-induced kidney injury) in the pathogenesis of acute renal failure is emphasized. We also analyze the impact of the acute renal failure in the lung, recognizing an increase in pulmonary vascular permeability, inflammation, and alteration of sodium and water channels in the alveolar epithelial. This conceptual model can be the basis for the development of new therapeutic strategies to use in patients with multiple organ dysfunction syndrome.

ABSTRACT

There is a lack of definitive information regarding the precise indications, implementation, and outcomes of continuous renal replacement therapy (CRRT) for the treatment of critically ill children. Six children (three boys, three girls) aged from 3 days to 8 years, all of whom had multiple organ failure, were submitted to bedside CRRT using M60 filter membranes. Modified Port carbonate formula was used and clotting time was maintained between 20 and 30 minutes. Activated partial thromboplastin time was 1.5- to 2-fold normal. One patient discontinued treatment due to family decision. Marked improvements were seen in the remaining five patients, including normalization of blood urea nitrogen and creatinine levels, stabilization of electrolytes, and improvements in markers of organ function. Of note, one patient (a six-year-old male) underwent the treatment for 241 hours. All five patients were subsequently discharged and recovered uneventfully. CRRT is effective for the management of children who are critically ill due to multiple organ failure.

ABSTRACT

Each year approximately $400 billion is spent treating Americans suffering some type of tissue loss or end-stage organ failure. This includes millions of dental and oral craniofacial procedure, ranging from tooth restorations to major reconstruction of facial soft and mineralized tissue. Recently, a population of putative post-natal stem cells in human dental pulp (DPSCs) has been identified within the "cell- rich zone" of dental pulp. The other type of stem cells from human exfoliated deciduous teeth (SHED) was identified to be a population of highly proliferative, clonogenic cells. Dental Pulp Stem Cells (DPSCs) can not only be derived from a very accessible tissue resource like SHED but are also capable of providing enough cells for potential cell-based therapies.

ABSTRACT

A thirty-six-year old female with shock was found to be unconsciousness a few days after developing a respiratory infection. Her past medical history included autoimmune hypothyroidism. Her state of shock was not controlled by massive fluid resuscitation with a vasopressor and antibiotics. However, an infusion of 250 mg methylprednisolone dramatically improved her shock state. Further examination indicated secondary acute adrenal insufficiency. Adrenal insufficiency may complicate other endocrine disorders. Accordingly, a physician should consider hypoadrenocorticism, when patients are in a state ofrefractory shock in spite ofmassive infusion with a vasopressor, especially in patients with other endocrine disorders.


Una mujer de treinta y seis años en shock fue hallada inconsciente unos dias después de desarrollar una infección respiratoria. Los antecedentes en su historia clinica incluian hipotiroidismo autoinmune. Su estado de shock no fue controlado por la reanimación con liquidos masiva con un vasopresor y antibióticos. Sin embargo, una infusion de 250 mg metilprednisolona habia mejorado considerablemente su estado de shock. Un examen mas detenido indicó insuficiencia adrenal aguda secundaria. La insuficiencia adrenal puede complicar otros trastornos endocrinos. En consecuencia, un médico debe considerar la posibilidad de hipoadrenocorticismo, cuando los pacientes se encuentran en estado de shock refractario a pesar de una infusion masiva con un vasopresor, especialmente en el caso pacientes con otros trastornos endocrinos.

A case of acute acalculous perforated cholecystitis with acute generalized peritonitis in a middle aged cachectic man, presenting late in a moribund condition, is reported. He underwent emergency laparotomy [subtotal cholecystectomy], went into multi-organ failure and was managed accordingly. The patient recovered in about 2 weeks and was followed-up regularly

FUNDAMENTO: A oxigenação por membrana extracorpórea (ECMO) em recém-nascidos e crianças tem resultados excelentes. A experiência em adultos é mais modesta e os resultados imediatos são inferiores. Entretanto, a sobrevida em cinco anos de pacientes que sobrevivem a esse método é bastante promissora. Não há relato na literatura brasileira de experiências com esse sistema nesse contexto. OBJETIVO: Relatar a experiência com o uso da oxigenação por membrana extracorpórea no adulto que se apresenta em choque cardiogênico agudo e refratário. MÉTODOS: Análise retrospectiva de prontuários médicos de pacientes submetidos a implante do sistema de oxigenação por membrana extracorpórea para a assistência circulatória no choque cardiogênico agudo e refratário. RESULTADOS: Onze pacientes (63,5 anos; 45,5 por cento homens) foram considerados para análise no período de 2005 a 2007. O tempo médio de suporte circulatório foi de 77 horas (10-240h) e cinco pacientes estavam vivos em 30 dias (45,5 por cento). Dois pacientes foram subseqüentemente submetidos a implante de assistência circulatória prolongada após um período de ressuscitação em ECMO, um dos quais foi submetido ao transplante cardíaco. As causas de morte durante a assistência com ECMO (seis pacientes) incluíram falência multiorgânica (66,6 por cento) ou sangramento refratário (33,4 por cento). CONCLUSÃO: O sistema ECMO é uma opção de assistência circulatória temporária para pacientes adultos com falência cardíaca aguda refratária, podendo ser utilizado como ponte para a recuperação ou com o intuito de selecionar candidatos à assistência circulatória prolongada (ponte para a ponte).


BACKGROUND: The extracorporeal membrane oxygenation (ECMO) has been used in the neonatal and childhood periods with excellent results. The adult experience has been modest with inferior immediate results. The intermediate survival, however, has been promising. We have been using the extracorporeal membrane oxygenation for temporary mechanical circulatory support of adults that present with acute refractory cardiogenic shock in our institution. There is no other published experience of the use of this system in this scenario in Brazil. OBJECTIVE: To describe our experience with the use of the extracorporeal membrane oxygenation for circulatory support in adults. METHODS: Retrospective analysis of the medical files of patients submitted to the implant of extracorporeal membrane oxygenation system for circulatory assistance in acute and refractory cardiogenic shock. RESULTS: Eleven patients (63,5yo; 45,5 percentmale) were considered for analysis from 2005 to 2007. Median support time was 77 hours (10-240h) and 5 patients have survived 30 days (45,5 percent). Two patients were subsequently submitted to prolonged paracorporeal circulatory assistance. Mortality on ECMO (6 patients) was due to multiple organ failure (66,6 percent) and refractory bleeding (33,4 percent). CONCLUSION: ECMO system is an option to be used in acute refractory cardiogenic shock as a bridge to recovery or selecting patients that might benefit from prolonged paracorporeal assist devices (bridge to bridge).

ABSTRACT

OBJECTIVES: This study aimed to assess the impact of the duration of organ dysfunction on the outcome of patients with severe sepsis or septic shock. METHODS: Clinical data were collected from hospital charts of patients with severe sepsis and septic shock admitted to a mixed intensive care unit from November 2003 to February 2004. The duration of organ dysfunction prior to diagnosis was correlated with mortality. Results were considered significant if p<0.05. RESULTS: Fifty-six patients were enrolled. Mean age was 55.6 ± 20.7 years, mean APACHE II score was 20.6 ± 6.9, and mean SOFA score was 7.9 ± 3.7. Thirty-six patients (64.3 percent) had septic shock. The mean duration of organ dysfunction was 1.9 ± 1.9 days. Within the univariate analysis, the variables correlated with hospital mortality were: age (p=0.015), APACHE II (p=0.008), onset outside the intensive care unit (p=0.05), blood glucose control (p=0.05) and duration of organ dysfunction (p=0.0004). In the multivariate analysis, only a duration of organ dysfunction persisting longer than 48 hours correlated with mortality (p=0.004, OR: 8.73 (2.37-32.14)), whereas the APACHE II score remained only a slightly significant factor (p=0.049, OR: 1.11 (1.00-1.23)). Patients who received therapeutic interventions within the first 48 hours after the onset of organ dysfunction exhibited lower mortality (32.1 percent vs. 82.1 percent, p=0.0001). CONCLUSIONS: These findings suggest that the diagnosis of organ dysfunction is not being made in a timely manner. The time elapsed between the onset of organ dysfunction and initiation of therapeutic intervention can be quite long, and this represents an important determinant of survival in cases of severe sepsis and septic shock.

ABSTRACT

La lesión tisular provoca dos tipos de respuesta inflamatoria: una local, cuya finalidad es el restablecimiento del bienestar tisular y otra, en la que existe ampliación o generalización de la inflamación ; con el compromiso de múltiples órganos. Se presenta el caso de un hombre joven, mestizo, agricultor procedente del área rural de Mitú (Vaupés) quien padeció de una picadura de avispa en la extremidad inferior derecha. Recibió tratamiento con varios fármacos como analgésicos, antieméticos y antibióticos, desarrollando fiebre, disnea, enantema, lesiones cutáneas en diana y disfunción multiorgánica. La histopatología fue acorde con toxidermia medicamentosa. El paciente recibió soporte hemodinámico y ventilatorio recuperándose plenamente

Substantial clinical and animal evidences indicate that the mesenteric circulatory bed, particularly the gut mucosa, is highly vulnerable to reductions in oxygen supply and prone to early injury in the course of hemodynamic changes induced by sepsis and septic shock. Gut hypoxia or ischemia is one possible contributing factor to gastrointestinal tract barrier dysfunction that may be associated with the development of systemic inflammatory response and multiple organ dysfunction syndrome, the principal cause of death after sepsis. Monitoring gut perfusion during experimental and clinical sepsis may provide valuable insights over new interventions and therapies highly needed to reduce multiple organ dysfunction and sepsis-related morbidity and mortality. We present our experience with gas tonometry as a monitor of the adequacy of gastrointestinal mucosal perfusion in experimental models sepsis and with the use of vasoactive agents for hemodynamic management in patients with septic shock.

ABSTRACT

La hemofiltración es utilizada con cada vez mayor frecuencia en pacientes pediátricos, en un variado número de situaciones clínicas. Se describe retrospectivamente una experiencia de 34 meses con este método aplicado a 17 pacientes, 9 varones edad promedio de 5 años 3 meses. 16 con falla orgánica múltiple, cinco con meningococcemia y riesgo (promedio) de mortalidad de 66 porciento. Se empleó hemofiltración arteriovenosa en 13 y venovenosa en 4 casos,asociada con diafiltración en 12 de los pacientes. Su indicación se debió siempre a edema y oliguria. En todos los casos se empleó heparina sistémica. No se registraron complicaciones de importancia. 9 pacientes (53 porciento) fallecieron debido a la afección de base, incluídos los 5 con meningococcemia y choque. La hemofiltración es un método práctico en pacientes pediátricos en situaciones críticas y puede contribuir a mejorar su manejo

ABSTRACT

Se presenta el caso clínico de una mujer de 30 años de edad, que durante la semana 35 de su tercer embarazo, desarrolló hígado graso agudo del embarazo. A pesar de una conducción y atención temprana y sin complicaciones del parto, la paciente presentó insuficiencia hepática fulminante y pancreatitis en el puerperio inmediato, cuando se esperaba su recuperación. La paciente ingresó a la unidad de cuidados intensivos en donde recibió tratamiento farmacológico y electromecánico temporal de apoyo para las insuficiencias orgánicas agudas que presentó. Debido a hiperbilirrubinemia extrema y coma hepático persistente, la paciente fue sometida a plasmaféresis. La paciente se recuperó completamente. Nuestro caso contrasta con reportes previos que informan sobre la resolución clínica espontánea y gradual de la hepatopatía una vez terminado el embarazo

ABSTRACT

Se estudiaron 51 pacientes internados en una unidad de terapia intensiva pediátrica. En ellos se hizo una valoración clínica derivada del índice de falla orgánica múltiple, que fue adaptada a las facilidades del hospital; ésta se identificó como índice terapéutico fisiológico (ITF). Se formaron dos grupos: uno con 14 pacientes que fallecieron y otro con 37 que sobrevivieron. Todos los pacientes fueron su propio control ya que se hicieron dos valoraciones: inicial y final. Resultados. La calificación inicial del índice en el grupo que sobrevivió fue de 18.3 ñ 6.9 y en el que falleció de 43.07 ñ 10.75, con una diferencia estadísticamente significativa (t = 1.9, p< 0.05). Las calificaciones finales del ITF en el grupo que sobrevivió fueron de 7.7 ñ 3.7 y para el grupo que falleció de 47.9 ñ 12.03. Al comparar las calificaciones inicial y final, éstas aumentaron; se encontró que en el grupo de pacientes fallecidos, el puntaje de calificaciones fue (t = 2.9 y p< 0.01). Los que sobrevivieron tuvieron una disminución de la calificación con respecto a los valores iniciales, (t = 10.27, p< 0.01). Se encontró una correlación entre la calificación de ITF y el número de fallas orgánicas. Conclusiones. El ITF discriminó desde la valoración inicial entre los pacientes que sobrevivieron y los que fallecieron. El valor crítico pronóstico fue de 37; los que fallecieron incrementaron su calificación y los que sobrevivieron la disminuyeron. Se propone una calificación límite de 20 para ingreso a una preterapia
